List all activities you participated in and honors and awards you received throughout freshman year. List all colleges and universities to which you were admitted, including those you do not plan to attend. If a scholarship was offered for any of these schools, please include the name of the scholarship and the dollar amount. For instance, UM – University Scholarship, $4000; MSU – Regent Scholarship, $500. If the scholarship is an annual scholarship for four years, please list the TOTAL amount of the scholarship, for instance, $25,000 per year would be $100,000.

This can be as general or as specific as you want it to be, for example – I plan to graduate in biology at Montana State University-Bozeman, attend med school at Washington University, become an orthopedic surgeon and live in Seattle, or – I am hoping to go into the field of medicine.
